Individual Membership

There is a one-time enrollment fee of $150 per DPC membership, regardless of the number of individuals enrolled.
DPC Membership is month-to-month, and you can cancel at any time.
When requested, cancellation will be effective the next monthly billing cycle.
Upon renewal, an enrollment fee equal of the initial enrollment fee current at the time of renewal applies, as well as the current memberhip pricing at that time.
Family members added at a later time are subject to the membership pricing current at the time of the addition, not at the time the membership was first started with the original member.

External Ancillary Services

There might be a separate lab charge for analysis of Pap smear sample or skin biopsy sample.

Limitation of Telemedicine Services
Services are restricted to when you are physically located in the state of Texas.
When you are outside the state of Texas, services are limited to phone calls, portal or secure text messages.

Frequency of Consultations

There are no restrictions on frequency, types, or duration of office consultation per month so long as membership is active.
There are never any co-payments for any consultations except for DOT Physicals.
Costs of medications and immunizations are not included, but provided at cost when available.
There are local resources available for low-cost immunizations, and routine screening tests such as mammogram and colonoscopy.
Membership pricing is subject to change at any time without notice.

It is recommended that you have certain types of catastrophic coverage in case of serious illnesses requiring emergency care or hospitalizations.

Medical Cost Sharing (also known as Health Cost Sharing)

Health Cost Sharing pricing is dependent on the plan or tier chosen.

Alternative for Health Cost Sharing plans are high-deductible, private, or short-term health insurance plans.
You may get a stipend or discount of your monthly health cost sharing plan contributions with Sedera Health, Zion Health, Liberty Healthshare, or Samaritan Ministries.
The Texas Department of Insurance lists all individual ACA-compliant insurance plans in Texas, both on and off the Exchange.

Generic (non-controlled) Prescription Medications are dispensed by Ochna Health Pharmacy and not included in membership.
For in-office procedures, an external lab charge may apply for analysis of skin or specimen samples.
For injections of medications, there is an extra charge for the medications themselves.
